Key Responsibilities\n

• Conduct thorough initial assessments with clients to determine their goals, preferences, and rehabilitation options, facilitating the development of customized treatment strategies.

• Design and implement targeted rehabilitation plans that are tailored to each client's unique educational background, skills, and vocational aspirations, adjusting plans as necessary to reflect their evolving needs.

• Engage closely with a multidisciplinary team, including physicians and occupational therapists, to collaboratively create and execute effective rehabilitation programs that cater to client-specific requirements.

• Maintain accurate and detailed case documentation, including personal information, insurance data, treatment histories, and progress reports, to ensure continuity of care and meet legal standards.

• Monitor clients’ progress regularly, documenting milestones and outcomes to evaluate success and identify areas for adjustment in their treatment plans.

• Provide ongoing support to clients during job training and occupational placement activities, addressing any challenges that arise promptly and effectively.

• Incorporate family and community resources into treatment plans, fostering a cooperative approach to meet clients' needs comprehensively.

• Assess client requirements and generate timely reports and recommendations for additional support as needed.

You will work primarily with a diverse client population that includes individuals grappling with various mental health issues, such as anxiety, depression, trauma, and substance abuse disorders.
