Key Responsibilities

As a Physical Therapist in our home care setting, your daily responsibilities will include:

• Evaluating patients referred by physicians to determine their physical therapy needs, establishing individual goals, and creating comprehensive treatment plans tailored to each patient.
• Implementing treatment interventions using a variety of techniques that target conditions such as orthopedic injuries, neurological disorders, and pediatric issues.
• Monitoring patient progress, modifying treatment plans as necessary, and utilizing evidence-based approaches to achieve optimal outcomes.
• Providing education and training for patients and their caregivers to facilitate understanding of conditions and empower them in their treatment journey.
• Documenting each patient's progress and maintaining accurate records using our electronic health records system, ensuring compliance with healthcare regulations.
• Collaborating with interdisciplinary teams and participating in team meetings to share insights, discuss patient care, and enhance the overall effectiveness of our services.
• Meeting key business objectives and utilization goals while continuously striving to improve patient satisfaction and clinical outcomes.

Patient Population / Client Focus

Our clients include a diverse range of individuals, from children needing care for developmental delays to elderly patients recovering from surgery or managing chronic conditions. Qualifications

To be considered for this role, you must possess the following qualifications:

• A Doctorate degree in Physical Therapy (DPT) from an accredited institution.
• A valid physical therapy license in the state of Florida.
• Completion of an APTA or CAPTE-approved physical therapy program.
• A current Basic Life Support (BLS) certification from the American Heart Association.
• Exceptional written and verbal communication skills, along with proficiency in using electronic medical records and standard office software.
• A genuine passion for helping others, coupled with a strong desire to establish and maintain positive relationships with patients and their families.

Preferred Experience

While a minimum of two years of experience in a physical therapy setting is required, candidates with a background in home care or experience working with diverse patient populations will be given preference.
