Rhythm & Motion Physical Therapy and Wellness is actively seeking a dedicated and licensed Orthopedic Physical Therapist to join our team as a Full-Time Home Care Specialist in Freehold, New Jersey. Work Environment

This Full-Time position entails working onsite in the comfort of patients' homes, allowing for personalized care that adapts to the unique challenges each patient faces. Operating within a model of home care presents a unique opportunity to build strong therapeutic relationships while delivering care tailored to the patients' real-life environments. The typical work schedule is weekdays from 8:00 am to 5:00 pm, Monday through Friday.

Key Responsibilities

• Interpret and comply with state practice acts and laws governing physical therapy, ensuring professional standards are met at all times.

• Conduct thorough assessments to evaluate patients with functional limitations and disabilities, formulating individualized treatment plans based on their unique needs and goals.

• Implement treatment protocols effectively that are informed by evaluation findings and ongoing patient assessments, adjusting plans as necessary based on patient progress.

• Maintain clear communication with other healthcare professionals to ensure a holistic approach to patient care, consulting when necessary to modify treatment plans for optimal outcomes.

• Document patient evaluations, daily encounters, progress summaries, and discharge evaluations with precision and timeliness to support clinical and billing operations.

• Participate actively in team meetings aimed at improving service delivery, addressing barriers to access, and enhancing the overall patient experience.

• Stay updated with ongoing training and compliance with corporate policies and relevant regulations at state, local, and federal levels.

Patient Population / Client Focus

Your primary focus will be on patients requiring orthopedic rehabilitation services. This may include post-operative care, injury recovery, chronic pain management, and functional restoration, particularly within the home environment where therapeutic exercises and functional activities are integrated into daily living contexts. Your expertise and interventions will play a crucial role in tailoring rehabilitation strategies that empower patients to regain independence and enhance their overall well-being.

Qualifications

• A Doctorate of Physical Therapy (DPT) from an accredited institution.

• Current state licensure as a Physical Therapist in New Jersey.

• A minimum of two years of outpatient orthopedic experience is required.

• Active CPR certification to ensure the safety and well-being of patients.

Preferred Experience

Preference will be given to candidates with prior home health experience and those who demonstrate a holistic understanding of rehabilitation in a community setting. Familiarity with electronic health record (EHR) systems, pain management protocols, and evidence-based practice will further enhance your ability to succeed in this role.

Compensation and Benefits
