Work Environment

This position is based onsite at our modern facility, which is equipped to provide a comprehensive range of medical services. The work schedule typically spans Monday through Friday, with hours from 9:00 a.m. to 6:00 p.m., and occasional evening clinics until 8:00 p.m. Participation in community health fairs and outreach events during weekends may also be required. You will be supported by a dedicated team of healthcare professionals in a collegial setting that values teamwork and mutual respect.

Key Responsibilities

• Perform thorough health assessments and screenings for patients of all ages, ensuring all factors affecting health are taken into account.
• Diagnose, treat, and manage a variety of acute and chronic medical conditions, focusing on preventive care and overall health promotion.
• Collaborate with community organizations to develop and deliver effective health education programs addressing prevalent community health issues.
• Provide culturally competent care to respect and accommodate the diverse backgrounds of your patients, advocating for their rights and improved access to care.
• Coordinate referrals and ensure follow-up appointments to maintain continuity of care for your patients.
• Maintain detailed patient records in compliance with healthcare regulations, facilitating the tracking and reporting of health outcomes.
• Engage in community outreach efforts such as health fairs, providing screenings and health education to promote wellness.
• Stay informed about the latest community health trends and evidence-based medical practices to enhance the quality of care provided.

Patient Population / Client Focus

Your primary focus will be on serving families and individuals from diverse socioeconomic backgrounds, emphasizing care for vulnerable populations. You will play a key role in addressing barriers to healthcare access and ensuring that all patients receive the high-quality care they deserve.

Qualifications

• Current certification as a Nurse Practitioner from an accredited program.
• Active and unrestricted Nurse Practitioner license in the state of Texas.
• Bachelor's degree in Nursing; a Master's degree is preferred.
• A minimum of 3-5 years of clinical experience, ideally in community health, family practice, or public health settings.

Preferred Experience

• Demonstrated experience working with multicultural populations and a commitment to health equity.
• Proficiency in using electronic health record (EHR) systems to manage patient data efficiently.
• Previous involvement in health education programming or community outreach initiatives.

Compensation and Benefits

We offer a competitive salary ranging from USD 120,000 to USD 220,000 annually, commensurate with experience and qualifications.
