Work Environment

This role will take place in an onsite assisted living facility, where the nurse practitioner will engage closely with residents and their families. The work environment is collaborative, supportive, and focused on fostering a positive atmosphere for both patients and staff. Our hours of operation are typically from 8:00 a.m. to 5:00 p.m., with occasional evening clinics until 8:00 p.m. and participation in weekend events as necessary. This allows for exposure to a variety of patient interactions and community engagement initiatives, averaging 40 hours of work per week.

Key Responsibilities

As an experienced Family Nurse Practitioner at Barcino Health Care Inc., your responsibilities will include:

• Conducting thorough health assessments and screenings for residents, facilitating early detection and intervention for health issues.
• Diagnosing and managing both acute and chronic medical conditions, providing essential preventive care and health promotion for patients.
• Collaborating with community organizations and healthcare stakeholders to design and deliver effective health education programs tailored to the unique needs of our patients.
• Ensuring culturally sensitive care by advocating for patients' rights and improving their access to healthcare services.
• Coordinating referrals and follow-up care, fostering continuity of care for residents and mitigating health disparities.
• Maintaining precise and detailed medical records that comply with all regulatory standards.
• Participating in community outreach initiatives and health fairs to elevate awareness of health issues and provide essential health screenings.
• Staying informed about current trends in community health, evidence-based practices, and applicable research to enhance patient care.

Patient Population / Client Focus

The Family Nurse Practitioner will have the unique opportunity to serve a diverse patient population, primarily made up of seniors residing in the assisted living facility. This demographic often requires specialized care management due to chronic conditions and complex health needs. Qualifications

To be considered for this role, candidates must possess the following qualifications:

• Current certification as a Nurse Practitioner.
• An active nursing license in the state of Texas.
• A Master’s degree in Nursing from an accredited institution.
• A minimum of 3-5 years of clinical experience, ideally within community health or public health settings.
• Strong interpersonal and communication skills, with proficiency in working effectively with diverse populations.

Preferred Experience

Compensation and Benefits
