This full-time, onsite role is integral in providing comprehensive healthcare services to our patients. Work Environment

The position is based at our home care facility in Corpus Christi, where you will collaborate with a team of healthcare professionals dedicated to providing personalized care. Our facility is designed to foster a collaborative work environment free from the pressures of on-call duties and weekend shifts. The standard work hours are Monday, Wednesday, and Friday from 8:00 a.m. to 5:00 p.m., allowing for a balanced work-life integration.

Key Responsibilities

• Conduct comprehensive primary care services, including thorough health assessments and evaluations tailored to individual patient needs.
• Perform onsite health risk assessments to identify potential health issues and develop strategic intervention plans.
• Educate patients on wellness, preventive measures, and healthy lifestyle choices, equipping them with the tools they need to succeed in maintaining their health.
• Support and guide individuals in making positive lifestyle changes, leveraging motivational interviewing techniques when necessary.
• Assist patients in managing chronic conditions by providing customized treatment plans and regular monitoring for effective disease management.
• Oversee and coordinate care provided by the medical staff within the home care setting, ensuring consistent and high-quality patient care.
• Foster collaborative relationships with the center's multidisciplinary team to enhance the overall patient experience and health outcomes.
• Promote community health initiatives by actively participating in wellness events and health fairs designed to educate and engage the population.

Patient Population / Client Focus

As a Family Nurse Practitioner with us, you will serve a diverse patient population with varying health needs. Your role will involve working closely with individuals across different age groups, emphasizing preventive care, chronic disease management, and health education to foster a healthier community.

Qualifications

• A Master's degree in Nursing with a specialization as a Family Nurse Practitioner.
• A minimum of 1 year of experience practicing as a Family Nurse Practitioner in a primary care or family practice setting.
• Must possess a current and unrestricted state nurse practitioner license.
• Proficient in utilizing Electronic Medical Record (EMR) systems to maintain accurate patient documentation.

Preferred Experience

• Experience in health risk assessment and management of chronic conditions.
• Demonstrated ability to build trust and engage with patients and their families effectively.
• Willingness to actively participate in community health fairs and wellness events to promote public health.

Compensation and Benefits

We offer a competitive salary range of USD 125,000 to 140,000 per year, commensurate with experience and qualifications. In addition to a dynamic and fulfilling work environment, employees who work 24 hours or more per week receive a comprehensive benefits package that includes:
