Key Responsibilities\n

• Conduct comprehensive assessments to accurately identify and address the unique needs of each patient.

• Maintain clear and constructive communication with supervisors, direct care staff, family members, and other relevant parties about patient progress and treatment plans.

• Create and share a tentative schedule that includes appointments, home visits, and observations to ensure consistent monitoring of each patient's progress.
\n
• Coordinate home visits to fulfill necessary assessments, such as observations, chart reviews, and staff interviews, essential for data analysis and treatment evaluation.

• Implement and ensure the effective execution of treatment plans, adjusting strategies based on ongoing evaluations of patient needs.
\n
• Engage actively in Interdisciplinary Team Meetings, providing insights and updates on shared patients to enhance collaborative care.
\n
• Complete detailed monthly and quarterly progress notes that document treatment outcomes and inform future care strategies.
\n
• Organize and lead training sessions to enhance the competence of staff in implementing treatment protocols and documentation practices.
\n
• Follow up with patients through interviews and observations during home visits to ensure continuity of care and support.
\n
• Maintain comprehensive and accurate records to ensure compliance with regulations and to provide readily accessible patient information.

• Act as a liaison to the public and the broader mental health community, fostering relationships that support developmental goals.

• Assist in developing and refining training materials aimed at enhancing program effectiveness for individuals served.

Patient Population / Client Focus\n

You will work with a broad spectrum of clients, including adults and youth dealing with a variety of issues such as anxiety, depression, trauma, and substance misuse.
