Key Responsibilities\n

• Conduct holistic, comprehensive assessments to identify the emotional, psychological, and social needs of clients in hospice care.

• Develop and implement personalized treatment plans based on client assessments, ensuring that interventions are tailored to individual circumstances.

• Utilize evidence-based therapeutic frameworks to provide solution-focused interventions that promote coping and adjustment during a critical life stage.

• Maintain accurate, timely, and confidential documentation of services provided, adhering to HIPAA and agency standards.
