Work Environment

As a Home Care Physical Therapist, you will primarily work onsite, providing services directly to patients in their own homes. This setting allows for a more personalized approach to treatment, accommodating patients' specific environments and promoting comfort during rehabilitation. You will be part of a supportive team, working closely with other healthcare professionals and family members to ensure continuity of care and optimal patient recovery.

Key Responsibilities

• Conduct comprehensive patient evaluations within 24 hours of physician referral, documenting any delays clearly in medical records, in accordance with state regulations.
• Develop tailored treatment plans based on individual patient assessments, and secure approval from referring physicians before implementation.
• Provide therapeutic interventions as per the established treatment plans, utilizing various physical therapy techniques to enhance mobility and function.
• Supervise and guide Physical Therapy Assistants in delivering patient care, ensuring adherence to state practice act regulations and best care practices.
• Engage in effective communication with multidisciplinary team members, family members, and supervisors on patient progress and any challenges encountered.
• Participate in patient care conferences, rehabilitation meetings, and utilization reviews to contribute to a seamless care continuum.
• Deliver ongoing in-service training sessions for facility staff to enhance their knowledge and skills related to patient care and therapy techniques.
• Maintain accurate and timely documentation of evaluations, treatment notes, and progress reports, aligned with the standards set by governmental regulations and third-party payers.
• Provide education to patients and their families about home exercise programs and necessary adjustments for a smooth transition post-therapy.
• Assist in discharge planning processes and recommend durable medical equipment, such as walkers and braces, to support patient independence post-treatment.
• Monitor and report any concerns regarding therapy equipment to ensure it remains in optimal working condition for effective service delivery.

Patient Population / Client Focus

The Home Care Physical Therapist will primarily work with diverse patient populations, including elderly individuals, post-surgical patients, and those recovering from acute or chronic conditions. Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited program.
• A current Florida State Licensure to practice as a Physical Therapist (PT).

Preferred Experience

Compensation and Benefits

The position offers a competitive salary ranging from $90,000 to $100,000 annually, commensurate with experience.
