Work Environment

This position is onsite, working directly with patients in their homes throughout Fort Myers. Our Home Care facility focuses on providing personalized and compassionate care within the comfort of our patients' living environments. The work schedule typically consists of 40 hours per week, with variable on-call requirements, including weekends, to accommodate patient needs.

Key Responsibilities

• Conduct thorough evaluations and assessments of patients to develop personalized treatment plans addressing their unique needs.
• Implement therapeutic interventions, monitor progress, and make adjustments to treatment plans as needed based on patient response and clinical guidelines.
• Maintain accurate and timely documentation of patient progress and treatment outcomes in compliance with regulatory standards.
• Work collaboratively with a multidisciplinary team including physicians, occupational therapists, and nursing staff to coordinate comprehensive patient care.
• Educate patients and their families about treatment plans, rehabilitation goals, and strategies to facilitate recovery and improve functional independence.
• Participate in community outreach and leadership activities aimed at promoting physical therapy services and enhancing community health.
• Attend and actively engage in weekly staff meetings and continuing education opportunities to further professional development and improve service delivery.

Patient Population / Client Focus

Our Physical Therapists work with a diverse population of patients, including those recovering from orthopedic surgeries, neurological conditions, and chronic illnesses. Each therapist is trained to address the specific needs of different age groups and cultural backgrounds, ensuring that all patients receive individualized and culturally competent care. Qualifications

• Bachelor's Degree in Physical Therapy from an accredited institution is required.
• Active and valid state license in Physical Therapy.
• Minimum of 2 years of experience as a practicing Physical Therapist, preferably in a home care or similar settings.
• Strong verbal and written communication skills are essential for effective collaboration and patient education.
• Proficiency in word processing and electronic health record systems.

Preferred Experience

While a minimum of 2 years of experience is required, candidates with experience in home health care, familiarity with electronic documentation systems, or specialized certifications in areas such as geriatric or orthopedic therapy will be preferred. Compensation and Benefits

This full-time position offers a competitive salary ranging between $100,000 and $120,000 annually, commensurate with experience.
