Key Responsibilities

• Conduct thorough assessments of patients' physical capabilities and limitations in their home environment.
• Develop and implement individualized treatment plans using evidence-based practices, including therapeutic exercises, manual therapy, and functional training to alleviate pain and enhance mobility.
• Utilize a variety of tools and modalities, adjusting techniques based on patient progress and preferences.
• Regularly track and document treatment outcomes, refining and adapting treatment plans to meet evolving patient needs.
• Educate patients and their families on techniques to manage their conditions and promote self-care, while also providing guidance on the use of any recommended assistive devices.
• Participate in clinical case discussions with other members of the healthcare team to ensure coordinated and comprehensive care for all patients.
• Engage in professional development by attending workshops, reading relevant literature, and networking with peers to maintain licensure and enhance skills.

Patient Population / Client Focus

Your focus will encompass a diverse patient population, including children, adults, and seniors facing various physical challenges caused by surgery, trauma, chronic illness, or age-related issues. Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited program.
• Current and valid physical therapy license in the state of Florida.
• Strong verbal and written communication skills to effectively interact with patients, families, and healthcare teammates.
• Proficiency in using electronic health records and basic computer applications.
• Flexibility to adapt to changing work environments and patient conditions.
• Commitment to ongoing professional development and lifelong learning.

Preferred Experience

• A minimum of two years of experience as a practicing physical therapist is preferred.
• Experience in a home health setting or knowledge of home care methodologies will be advantageous, but not required.
