Work Environment\n

This role will primarily be conducted remotely, allowing for flexibility while maintaining a structured work schedule from Monday to Friday, 9:00 AM to 5:00 PM. Key Responsibilities\n

• Maintain close communication with physicians and other healthcare professionals to ensure comprehensive patient care by reporting on patient progress, needs, and any concerns related to their psychological state.

• Document clinical and progress notes accurately and timely, aligning with departmental protocols to ensure that all patient information reflects the most up-to-date status of their treatment.

• Collaborate with healthcare team members to develop effective treatment plans and discharge strategies based on psychosocial assessments and established evidence-based social work interventions.

• Provide individualized counseling and crisis intervention, assessing emotional and behavioral challenges and implementing tailored treatment strategies to address these issues.

• Establish and nurture relationships with community resource leaders and stakeholders, representing the organization’s interests in meetings and committees that focus on social services for our patients.

Qualifications\n

• A Master’s degree in Social Work, licensure, and certification as a Licensed Clinical Social Worker (LCSW) are required.

• Current Associate or Licensed status with the Board of Behavioral Sciences (or actively in progress).
