Key Responsibilities\n

• Conduct thorough ongoing assessments of clients to evaluate their needs and formulate personalized treatment plans.

• Maintain open communication with supervisors, direct care staff, and client families regarding treatment progress, ensuring a collaborative approach to care.

• Organize a tentative schedule for appointments, home visits, and observations to monitor clients’ advancements effectively.
\n
• Facilitate home visits to gather observational data, conduct chart reviews, and perform staff interviews to enhance service delivery.
\n
• Ensure the effective implementation of individualized program plans, adapting strategies as necessary to meet client goals.
\n
• Participate in interdisciplinary team meetings to discuss client progress and collaborate on integrated care plans.
\n
• Complete and maintain accurate monthly and quarterly progress notes following established documentation protocols.
\n
• Provide training and mentorship to staff, ensuring competence in program implementation and documentation practices.
\n
• Conduct follow-up interviews and additional home visits to ensure ongoing support for our clients’ evolving needs.

• Act as a liaison with the mental health community and public organizations to further develop support networks and community resources.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is required.
