Key Responsibilities\n

• Conduct comprehensive psychosocial assessments for individuals, families, and groups, tailoring care plans according to their unique needs.

• Provide therapeutic interventions, including Cognitive Behavioral Therapy (CBT) and other evidence-based modalities, aimed at enhancing clients’ emotional resilience and coping strategies.

• Collaborate actively with interdisciplinary teams through documentation and meetings, such as patient rounds, to ensure cohesive and integrated care.

• Develop and maintain therapeutic relationships, offering continuous support, guidance, and motivation to facilitate clients' progress.

• Oversee case management for assigned patients, connecting with community resources and professionals to deliver the most fitting care.

• Maintain timely and ethical documentation of all interactions and treatment plans, adhering to company standards and regulations.

Patient Population / Client Focus\n

Your focus will include a diverse group of clients ranging from adolescents to adults who seek mental health services in an outpatient setting. Many of our clients are facing various challenges including anxiety, depression, grief, and adjustment disorders typically associated with end-of-life care at our hospice center. Your role will significantly influence their coping strategies and overall mental health outcomes.

Qualifications\n

• A valid and current license as a Licensed Clinical Social Worker (LCSW) , Licensed Professional Counselor (LPC) , Licensed Marriage and Family Therapist (LMFT) , or Doctor of Psychology (PsyD) .

• A Bachelor's Degree in Social Work, Counseling, Psychology, or a related field.

• Minimum of 2 years of clinical experience in mental health or a related field.

Preferred Experience\n

• Demonstrable experience with a variety of treatment modalities, especially evidence-based practices like CBT.

• Proficiency in electronic health record (EHR) systems to facilitate effective management of client information.
