This position is fully remote, allowing you to work from the comfort of your home while still being part of a vibrant team. As a Licensed Independent Clinical Social Worker, you will frequently interact with healthcare professionals, including physicians and nurses at our Hospice Center in Marysville, Washington. Key Responsibilities\n

• Communicate effectively with medical professionals to report on patient progress, needs, and any concerns, ensuring comprehensive care and quick interventions when necessary.

• Maintain accurate and timely documentation, including clinical assessments and progress notes, adhering to departmental policies and standards.

• Provide immediate support to clients in crisis by responding to their emotional and psychological needs, and develop tailored intervention strategies for their safety and well-being.

• Utilize assessment tools and methodologies to regularly evaluate and document clients' progress towards their treatment goals, modifying plans as necessary based on their evolving needs.

• Consult and coordinate treatment plans with families and support systems to facilitate optimal patient care and discharge planning, ensuring all parties are informed and involved in the process.

Patient Population / Client Focus\n

This role focuses on individuals receiving hospice care who often require emotional support during a critical phase of their lives. You will work closely with patients facing complex health challenges and their families, providing guidance, counseling, and support in navigating this difficult time.

Qualifications\n

• A Master’s degree in Social Work, Counseling, Psychology, or a closely related field is required.

• You must hold an active state license as a Licensed Independent Clinical Social Worker (LICSW) or an equivalent.

• A minimum of three years of experience working in an outpatient setting is essential, particularly with patients in palliative or hospice care.

• The ideal candidate will be highly organized, self-directed, and capable of working independently.

• Proficient computer skills, including familiarity with electronic medical records, word processing, spreadsheets, and communication platforms, are necessary.

Preferred Experience\n

Experience in a hospice or palliative care setting is highly preferred. Familiarity with grief counseling and crisis intervention strategies will be advantageous as you provide compassionate care tailored to the needs of patients and their families.
