Key Responsibilities\n

• Conduct thorough intake and psychosocial assessments, gaining a comprehensive understanding of each patient’s life history, emotional state, and support needs.

• Develop and present detailed treatment plans to the Collaborative Dependency Recovery Program (CDRP) and support teams, tailoring interventions based on specific patient situations and feedback from interdisciplinary partners.

• Participate in regular clinical case conferences to discuss patient progress, share insights, and collaborate on treatment strategies with fellow professionals.

• Deliver educational workshops focused on chemical dependency, equipping patients and families with knowledge about addiction, recovery options, and available resources.

• Provide specialized chemical dependency counseling services, offering both individual sessions and group therapy to address substance use disorders effectively.

• Facilitate family and multi-family therapy sessions to promote healing and communication among loved ones affected by substance use-related issues.

• Offer case management support, coordinating resources and referrals to ensure patients access necessary services beyond therapy, such as medical care or community programs.

Your work will primarily focus on hospice patients and their families, many of whom are navigating complex emotional, psychological, and relational challenges. Qualifications\n
