Key Responsibilities\n

As a Licensed Professional Counselor, you will:

• Conduct thorough patient intakes, emphasizing diagnostic and functional evaluations to develop personalized treatment plans.

• Collaborate closely with primary care providers to identify, treat, and manage mental health and psychosocial issues, integrating behavioral health care into the primary care setting.

• Monitor patient progress, initiating follow-ups to assess effectiveness of treatment plans and making adjustments as necessary.

• Document all treatment progress and diagnostic information diligently in treatment charts.

Compensation and Benefits\n
