Key Responsibilities\n

• Conduct comprehensive and ongoing assessments to identify and address the specific mental health needs of your clients, informing the treatment plans accordingly.

• Facilitate regular communication between supervisors, co-workers, and the clients’ support networks—including family members and guardians—to monitor progress effectively.
\n
• Develop and maintain a tentative schedule of appointments, home visits, and necessary observations to ensure effective client follow-up.
\n
• Coordinate and execute home visits to perform in-home observations, review documentation, and engage in data analysis for informed decision-making.
\n
• Implement treatment plans with fidelity, regularly assessing efficacy and making necessary adjustments to improve client outcomes.
\n
• Attend interdisciplinary team meetings to collaborate on client care strategies, sharing insights and perspectives to enhance collective understanding.
\n
• Document monthly and quarterly progress notes, ensuring comprehensive tracking of client growth and challenges.
\n
• Organize and lead training sessions for staff, ensuring compliance with established program plans and documentation standards for improved care delivery.
\n
• Provide ongoing support through follow-up interviews, home visits, and additional observations, dedicated to meeting client needs.
\n
• Act as a liaison between clients and the broader mental health community, representing their interests and advocating for their needs.
\n
• Assist in the development of procedures and training materials to enhance program effectiveness and improve delivery to clients.

Patient Population / Client Focus\n

In this role, you will work with a diverse client population, including both adults and youth experiencing a range of mental health issues. Your expertise in counseling will help address various problems, such as anxiety, depression, trauma, and substance misuse, fostering a supportive environment that encourages personal growth and healing.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a closely related field is required.

• Active and unrestricted licensure as a Professional Counselor (LPC) in New Jersey.
\n
• A minimum of two years of clinical counseling experience, demonstrating your capability in a therapeutic setting.

• Proficiency in family therapy, couples therapy, substance misuse, or trauma-informed care is highly preferred.

• A strong understanding of ethical and legal standards governing mental health practice.
\n
• Exceptional interpersonal skills, with the ability to establish rapport with a diverse array of clients.

• Demonstrated capability to collaborate effectively within a multi-disciplinary healthcare team.

• Familiarity with Electronic Medical Records (EMR) software and modern technology tools used in healthcare delivery.
