Key Responsibilities\n

As a Licensed Clinical Social Worker, you will be tasked with:

• Conducting thorough biopsychosocial assessments for patients in a hospice setting, evaluating their mental health, substance use, and medical needs.

• Providing individual and group therapy sessions to patients and families, using evidence-based therapeutic techniques to facilitate emotional healing and coping strategies.

• Documenting mental health evaluations, progress notes, and risk assessments electronically, ensuring compliance with healthcare regulations and best practices.
\n
• Identifying and escalating patient needs to appropriate healthcare providers when necessary, ensuring timely and effective interventions.

• Collaborating with community resources to connect patients and families with necessary support services, such as medication assistance programs and grief counseling.

• Working with Quality Improvement teams to gather data for program evaluation and improvement initiatives.

Patient Population / Client Focus\n

This role primarily involves working with patients facing chronic illnesses and end-of-life issues, along with their family members. You will provide support to a diverse range of clients, ensuring that cultural backgrounds and individual preferences are respected throughout the caregiving process. This comprehensive approach helps to ease the psychological burdens of patients and enhances the overall quality of care provided by our hospice team.

Qualifications\n

The ideal candidate will possess:

• A Master’s degree in Social Work (MSW) and a current state license as a Licensed Clinical Social Worker (LCSW).

• At least three (3) years of post-licensure experience in a clinical or hospice setting, demonstrating engagement in direct patient care.

• Robust knowledge of counseling techniques, crisis intervention strategies, and the ability to apply independent judgment to effectively address patient needs.

• Proficiency in electronic health record (EHR) systems for documentation and reporting purposes.

Preferred Experience\n
