Key Responsibilities\n

• Provide both individual and group counseling services via telehealth, ensuring regular direct service hours aligned with patient schedules.

• Conduct comprehensive Bio-psychosocial assessments to understand each patient's unique mental health needs and strengths.

• Facilitate the creation and continuous review of individualized treatment plans, working collaboratively with patients to define their recovery goals.

• Develop aftercare plans and discharge processes that support sustained recovery outside the clinic.
\n
• Ensure the efficient and effective delivery of counseling services, using evidence-based methodologies that align with best practice standards.

• Participate actively in team meetings and participate in ongoing training and development opportunities.

• Perform additional duties as assigned by your supervisor to continuously improve service delivery and patient outcomes.

Patient Population / Client Focus\n

The patients you will serve at Wellness Bridge include diverse adults, children, and families, many of whom experience challenges related to chemical dependency and mental health disorders. You will be instrumental in creating a safe space where clients feel understood and valued, fostering open communication that promotes recovery. A positive, non-judgmental attitude is essential as you guide clients through their unique journeys of healing.

Qualifications\n

• A Master’s degree in Mental Health Counseling, Social Work, Marriage and Family Therapy, Psychology, or a closely related field.

• A valid license to practice as an LMHC, LCSW, LMFT, or PsyD in the state of New York.
\n
• A minimum of 2 years post-licensure experience in a clinical setting, with a strong emphasis on direct patient care.

• Experience working with a range of populations, particularly in providing marriage and family therapy services.

• Excellent communication and interpersonal skills, with the ability to engage clients and collaborate with multidisciplinary teams effectively.

• Self-motivated and adept at working independently while also contributing to team-oriented initiatives.

Compensation and Benefits\n
