Work Environment

As a Physical Therapist in our organization, you will work onsite primarily in a home care setting, conducting evaluations and treatment sessions in patients' homes. Key Responsibilities

• Conduct thorough Physical Therapy evaluations for patients, focusing particularly on pediatric and adult demographics to assess their individual rehabilitation needs.
• Develop and implement customized treatment plans that leverage evidence-based practices to aid recovery and functional improvement.
• Regularly reassess and modify treatment plans based on patient progress and clinical findings, ensuring that therapies remain effective and patient-centered.
• Document all aspects of patient care, from SOAP notes to treatment summaries, utilizing our electronic records system efficiently.
• Educate patients and their families about therapy processes, expected outcomes, and any challenges that they may encounter during rehabilitation.
• Collaborate with multidisciplinary teams within Physiocare to ensure comprehensive care for clients receiving multiple services.
• Monitor and strive to achieve utilization and performance goals as mandated by the organization.

Qualifications
