Key Responsibilities\n

• Conduct thorough assessments of patients' psychosocial needs, behavior, and capacities through interviews and direct observation, tailoring interventions to their unique circumstances.

• Design and implement individualized treatment plans, incorporating evidence-based practices and therapeutic modalities to address challenges related to mental health, such as anxiety, depression, grief, and coping with chronic illness.

• Facilitate individual therapy sessions, family therapy, and group counseling to promote healing and provide a supportive space for shared experiences among patients and family members.

• Maintain accurate and compliant medical documentation, ensuring that all patient interactions and interventions are recorded according to organizational policies and state regulations.

Patient Population / Client Focus\n

In this role, you will primarily serve patients facing end-of-life challenges, their families, and caregivers. Qualifications\n

• Master's degree in Social Work (MSW) is required, along with a valid and active LCSW license in the state of California.

Compensation and Benefits\n
