Key Responsibilities

• Provide high-quality, patient-centered care by managing the complete therapeutic process for an assigned group of patients.
• Conduct thorough patient assessments and develop individualized care plans that reflect the goals and preferences of each patient.
• Implement treatment interventions that adhere to both the physician's plan of care and established ethical standards.
• Evaluate patient progress and therapeutic outcomes, modifying care plans as necessary to ensure effective rehabilitation.
• Instruct patients and their families on therapies, exercises, and the use of therapeutic appliances to be utilized during and outside of therapy sessions.
• Prioritize caseload, determine daily staffing needs, and coordinate assignments to optimize patient care delivery.

Patient Population / Client Focus

In this role, you will work with a diverse patient population, primarily focusing on individuals requiring orthopedic rehabilitation. This includes patients recovering from surgeries such as joint replacements, fractures, and other musculoskeletal injuries. Your ability to provide compassionate care is critical in empowering patients through their recovery journey, enhancing their physical capabilities while addressing their emotional and psychological well-being.

Qualifications

• A Bachelor's Degree in Physical Therapy, with a progression towards obtaining a Doctor of Physical Therapy (DPT) Degree.
• Current state licensure as a Physical Therapist, ensuring compliance with regulatory standards.
• A clean disciplinary record, reflecting a strong commitment to ethical practice and professionalism.
• Proficient manual therapy skills and experience in developing personalized exercise regimens.
• Familiarity with electronic medical record (EMR) systems, such as WebPT, to ensure efficient documentation and patient tracking.

Preferred Experience

• At least two years of experience in an orthopedic or rehabilitation setting, highlighting your ability to manage diverse patient needs.
• Experience in home health care services, demonstrating adaptability to patient environments.
• A strong track record of achieving positive patient outcomes through targeted interventions and rehabilitation planning.

Compensation and Benefits
