Key Responsibilities\n

• Utilize established patient education programs, integrated social work theories, methods, and quality of life assessment tools to evaluate client needs and create tailored care plans.

• Facilitate individual and group counseling sessions, providing psychoeducation and therapeutic interventions to support clients coping with mental health challenges.

• Monitor and assess client psychosocial status, offering interventions to aid adjustment after significant health events, such as dialysis treatment, and improve their overall quality of life.

• Collaborate with interdisciplinary teams to identify and address barriers to treatment, enhancing patients' overall recovery experiences.
\n
• Assist clients in navigating community resources, such as home health services and vocational rehabilitation, to facilitate their recovery and support their employment or education aspirations.
\n
• Collaborate with clients to evaluate and discuss advance directives, ensuring they understand their rights and preferences in decision-making processes.
\n
• Participate in comprehensive patient assessments to determine strengths and areas needing support, pursuing an integrative approach to improve rehabilitation outcomes.

• Engage in goal-directed counseling for clients seeking transplants, equipping them with the tools and knowledge necessary for successful transitions.

Patient Population / Client Focus\n

This role primarily focuses on providing care to individuals facing various mental health issues, including but not limited to chronic illness management, life transitions, and emotional challenges. As a counselor, you will be instrumental in enhancing the clients' psychosocial well-being and supporting their coping mechanisms as they navigate through their treatment journeys.

Qualifications\n

• Valid social work license (LMHC, LMFT, LICSW) in compliance with state regulations.

• Master’s degree in Psychology, Counseling, Social Work, or a related field.

• Authorized to work in the U.S.

Compensation and Benefits\n
