Work Environment

As a member of our team, you will work primarily onsite within patients' homes, fostering a comfortable and familiar environment that optimizes recovery. This role requires traveling within Lehigh Acres and surrounding areas, bringing therapeutic interventions directly to patients. Key Responsibilities

• Conduct comprehensive evaluations and assessments of patients in their home settings, determining the most effective treatment strategies.
• Develop individualized treatment plans that incorporate various techniques, such as therapeutic exercises, mobility training, and manual therapy, in alignment with each patient's recovery goals.
• Document all treatments, responses, and patient progress meticulously, ensuring accurate and accessible records for follow-up and communication with other health care professionals.
• Continuously monitor and assess patient progress, adjusting treatment plans as necessary to improve outcomes and aid recovery.
• Educate patients and their families about rehabilitation strategies and health management, empowering them with the knowledge and tools necessary for long-term success.
• Mentor and train team members and caregivers in techniques and use of assistive equipment, enhancing the overall care environment.
• Participate in ongoing professional development and training workshops to stay updated on the latest practices and methodologies in physical therapy.

Patient Population / Client Focus

Our patient population includes a diverse range of individuals experiencing mobility challenges due to injuries, chronic illnesses, post-operative recovery, and age-related conditions. By working in their familiar home environments, our therapists provide care that is both personalized and sensitive to the needs of each individual, allowing for a more effective rehabilitation experience.

Qualifications

• A Bachelor's degree in Physical Therapy or Doctorate of Physical Therapy (DPT) from an accredited institution.
• A valid Physical Therapy license in the state of Florida.
• Proficient verbal and written communication skills, with the ability to interact effectively with patients and interdisciplinary team members.
• Strong organizational skills with a commitment to maintaining thorough patient records.
• A compassionate and patient-centered approach to care.
• Highly adaptable with the ability to work collaboratively in a dynamic environment.

Preferred Experience

• At least 2 years of experience in a clinical physical therapy setting, with preference given to those familiar with home care practices.
• Experience with a wide range of therapeutic modalities and techniques.
• Working knowledge of electronic health record (EHR) systems.
