Key Responsibilities\n

• Provide clinical assessments, therapeutic interventions, and ongoing support to clients experiencing major mental illness and/or severe emotional disorders.

• Collaborate effectively with an interdisciplinary team, working towards common goals and enhancing team initiatives that improve patient care.

• Conduct evidence-based screenings and assessments, determining the need for more extensive evaluations when necessary.
\n
• Develop integrated care plans tailored to the unique needs of clients, facilitating access to required services while ensuring proper communication among clients, families, and healthcare providers.

• Deliver crisis intervention services and support families in navigating complex emotional challenges during critical times.

• Maintain caseload management, ensuring comprehensive client documentation and progress reports are completed timely and accurately.

• Participate in quality improvement committees, contributing to the evolution of service delivery and the enhancement of care models.

Patient Population / Client Focus\n

The ideal candidate will interact primarily with clients confronting severe emotional challenges in a hospice care context. Many of these individuals require compassionate support as they and their loved ones cope with end-of-life issues. You will play a vital role in enabling clients to make informed choices and empowering them through their care journey by providing ongoing emotional support, crisis management, and guidance.

Qualifications\n

• A Master’s degree in Social Work (MSW) or a closely related field is mandatory for this position.

• Current LCSW licensure in the state of Texas is required.

• Expertise in assessment, diagnosis, treatment interventions, risk assessment, and crisis management.

• Exceptional verbal and written communication skills, essential for engaging with clients and collaborating with the team.

• Demonstrated organizational skills, allowing for efficient caseload management and documentation.
\n
• Competency in developmental and age-specific issues related to the populations served.

Preferred Experience\n

While a minimum of two years of clinical experience is preferred, we also value strong candidates with unique expertise or a passion for working in hospice and charitable care settings. Bilingual proficiency in English and Spanish is a plus, enhancing our ability to serve a diverse client base effectively.

Compensation and Benefits\n

We are proud to offer a competitive salary range of USD 80,000 to 100,000 annually, reflective of your experience and contributions.
