Work Environment

This role is primarily onsite and involves direct patient care in home settings. Working full-time from Monday to Friday across a 40-hour shift, you will enjoy a balance of professional growth and meaningful patient interactions. The Home Care setting allows therapists to implement tailored approaches in familiar environments, facilitating more effective rehabilitation and recovery processes.

Key Responsibilities

• Conduct thorough evaluations of patients referred by physicians, focusing on individual needs and functional goals.
• Design and implement customized treatment plans that address a diverse range of conditions, including orthopedic, pediatric, neurologic, and sports medicine cases.
• Deliver outstanding customer service and support to enhance the patient experience and contribute to clinic success.
• Collaborate with multidisciplinary teams to align care strategies, ensuring the best possible outcomes for patients using evidence-based methods.
• Perform skilled therapeutic interventions at patients' homes, utilizing various procedures and techniques to encourage positive changes in physical function aligned with the physician's guidance.
• Educate and train patients and their caregivers on processes, ensuring they are equipped to participate actively in their recovery journey.
• Meet established utilization goals and contribute to broader business objectives.

Patient Population / Client Focus

You will have the unique opportunity to work with a diverse range of patients, from children needing rehabilitation for sports injuries to elderly patients recovering from surgery. Qualifications

• Must hold a Bachelor's degree in Physical Therapy from an APTA or CAPTE-approved program.
• Possession of a valid State License in Physical Therapy is required.
• Basic Life Support (BLS) Certification from the American Heart Association must be current.
• Superior written and verbal communication skills to effectively convey treatment plans and collaborate with team members.
• Proficiency in word processing and computer skills to maintain accurate patient records and documentation.
• A commitment to building strong therapeutic relationships to help patients exceed their rehabilitation goals.
• A friendly, supportive demeanor and a genuine desire to connect with patients and their families.

Preferred Experience

• At least 2 years of direct experience in physical therapy, particularly in a home health or community-based setting is preferred.
• Prior experience working with diverse patient populations and various physical therapy specialties will be advantageous.

Compensation and Benefits
