Key Responsibilities\n

The responsibilities of the Licensed Professional Counselor include:

• Collaborating with primary care providers to identify, diagnose, and manage mental health and psychosocial issues among patients.

• Conducting thorough patient intakes, focusing on diagnostic assessments, functional evaluations, and making informed recommendations to the primary care team about treatment goals.

• Delivering brief, focused interventions for patients seeking mental health services, ensuring that their unique needs are addressed.

• Establishing follow-up protocols to monitor patient progress, adjusting treatment plans as necessary based on ongoing evaluations.

• Developing relapse prevention strategies and assisting patients in maintaining stable functioning.
\n
• Recognizing at-risk patients and crafting intervention plans to prevent deterioration of their mental health.

• Coordinating with other health service providers to ensure comprehensive care, maintaining clear communication while respecting patient confidentiality.

• Documenting patient progress in accordance with standards for telehealth services, and ongoing collaboration with primary care providers for effective treatment plans.

• Providing patients with educational information and self-management skills to facilitate active participation in their treatment and recovery.

• Consulting with clinic management and other teammates on behavioral services, contributing to program evaluation efforts.

• Assisting patients in adhering to medical treatments initiated by the primary care provider, offering support and guidance to manage treatment side effects.

• Maintaining compliance with the Code of Ethical Conduct for Clinical Professionals and performing other related duties as assigned.

Qualifications\n

To be considered for this role, candidates must possess:

Compensation and Benefits\n

The annual salary for this position ranges from USD 87,000 to USD 105,000, commensurate with experience and qualifications.
