Work Environment\n

This position is entirely remote, offering flexibility while working closely with a dedicated hospice care team. You will engage with clients primarily from home or on-site visits as necessary, providing care that aligns with our organizational mission. Our full-time work hours are set from Monday to Friday, from 8:00 AM to 4:30 PM, with occasional evenings and weekends required to meet clients’ needs effectively.

Key Responsibilities\n

• Provide individualized clinical social work services to hospice patients, conducting thorough assessments and developing targeted treatment plans.

• Engage in direct clinical practices both in virtual settings and, when necessary, through home visits, ensuring patients receive compassionate support during difficult times.

• Collaborate seamlessly with a multidisciplinary team, participating actively in case consultations and team meetings to enhance patient care strategies.

• Maintain ethical practices and uphold the standards and code of conduct established by Inspiring Possibilities Counseling Center.

• Foster relationships with families by providing resources, emotional support, and education to assist them during the end-of-life process.

• Utilize evidence-based practices for assessment, diagnosis, and therapeutic interventions to ensure high-quality mental health care.

• Document clinical findings and treatment outcomes per regulatory and organizational standards, using our electronic health records system proficiently.

Patient Population / Client Focus\n

Your primary focus will be on hospice patients who may exhibit severe emotional distress resulting from serious illnesses. You will work with individuals and families from diverse backgrounds, navigating complex emotions surrounding grief, loss, and terminal diagnoses. This role requires cultural competence and an ability to empathize with varied life experiences, providing tailored support that respects their traditions and beliefs.

Qualifications\n

• A Bachelor's degree in Social Work from an accredited institution, with a current LCSW license in California.

• Extensive knowledge in assessment, diagnosis, and treatment methodologies specifically for severe emotional disorders and crisis management.
