Work Environment\n

This position will primarily be remote, allowing you the flexibility to manage your work-life balance while still providing essential services from the comfort of your home. Your work hours will be Monday to Friday from 9:00 AM to 5:00 PM. Our team is collaborative, with regular virtual meetings and check-ins to ensure that all team members stay connected and supported. Our hospice center model focuses on a holistic approach to care, serving clients compassionately throughout their healthcare journey.

Key Responsibilities\n

• Act as a primary liaison between families, patients, and outpatient providers to ensure seamless communication and care continuity.

• Engage with insurance companies to facilitate pre-certification processes and manage utilization effectively, thereby supporting patient access to vital services.
\n
• Contribute actively to discharge planning and aftercare strategies, ensuring that each client has a comprehensive and supportive transition back to their living environment.
\n
• Provide therapeutic meal support, focusing on promoting a healthy relationship with food and nutrition, which is critical in the hospice context.
\n
• Deliver safe, empathetic, and affirming care tailored to the unique circumstances of each client, integrating their intersecting identities into the treatment approach.

Patient Population / Client Focus\n

You will primarily work with older adult patients facing life-limiting illnesses in a hospice care setting. This role requires an understanding of the emotional and psychological aspects of end-of-life care, and your contributions will be pivotal in addressing the holistic needs of both patients and their families. Providing compassionate care during challenging moments is central to improving client outcomes.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• A valid LCSW license in the state of Kansas is required.

• Demonstrated experience working with older adults, particularly within hospice or end-of-life care contexts.

Compensation and Benefits\n
