Key Responsibilities\n

• Conduct comprehensive clinical assessments for patients in hospice care, developing individualized treatment plans that cater to their specific emotional and social needs.

• Utilize a range of therapeutic modalities, including cognitive-behavioral therapy (CBT), dialectical behavior therapy (DBT), and solution-focused interventions, tailoring your approach to fit the unique circumstances of each client.

• Maintain accurate and timely clinical documentation, ensuring compliance with HIPAA regulations and standard protocols, while keeping the confidentiality and safety of patient information a priority.

• Collaborate closely with an interdisciplinary team to deliver holistic care strategies that integrate mental, emotional, and physical well-being, enhancing the quality of life for both patients and their families.

The ideal candidate will directly support patients undergoing hospice care, who often present with a diverse array of psychological and sociocultural backgrounds. Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Current licensure as a Licensed Clinical Social Worker (LCSW) in California.

Compensation and Benefits\n
