Key Responsibilities\n

• Conduct comprehensive assessments that accurately identify the unique needs of each patient across diverse backgrounds and circumstances.

• Maintain effective communication with supervisors and other healthcare professionals, including family members and guardians, to provide updates on client progress and engage in collaborative planning.

• Develop and present a structured schedule of appointments and home visits to monitor each client’s progression through their individual treatment plans.

• Coordinate home visits and in-home observations when applicable to ensure thorough evaluations are performed in natural environments, thereby enhancing the therapeutic process.

• Oversee and ensure the effective implementation of treatment plans, adjusting approaches based on client responses and interdisciplinary insights.

• Actively participate in interdisciplinary team meetings to discuss client care strategies, share best practices, and contribute to continuous program improvement.

• Document monthly and quarterly progress notes meticulously to support ongoing patient care and compliance with healthcare regulations.

• Facilitate training sessions for new staff and team members to ensure adherence to program specifications and effective documentation practices.

• Provide ongoing follow-up through interviews and sessions to empower clients as they navigate personal challenges with tailored support.

Qualifications\n

• Master's or Doctoral degree in Counseling, Psychology, Social Work, or a related field is required.
