Key Responsibilities\n

• Conduct in-depth mental health assessments, including psychosocial evaluations, diagnostic interviews, and comprehensive risk assessments, to identify patients' needs.

• Collaborate closely with patients to develop individualized treatment plans that specifically consider their emotional, psychological, and cultural needs.

• Utilize evidence-based therapeutic interventions such as Cognitive Behavioral Therapy (CBT), Dialectical Behavior Therapy (DBT), and Solution-Focused Brief Therapy (SFBT) across various formats, including individual, couples, family, and group sessions.

• Provide crisis intervention services, ensuring that acute cases receive immediate care and facilitating a smooth transition to continuous service.

• Maintain accurate clinical documentation, including progress notes, treatment plans, and discharge summaries, adhering strictly to clinic policies and relevant regulatory guidelines.

• Empower patients and their families by providing psychoeducation that informs and equips them to manage mental health challenges effectively.

• Engage in regular outcome assessments and treatment effectiveness evaluations, adjusting therapeutic approaches based on individual progress.

• Stay abreast of current research, best practices, and emerging trends in mental health, integrating these insights into your clinical practice.

• Uphold the highest ethical standards, ensuring confidentiality and respect for each patient’s information.

Patient Population / Client Focus\n

You will primarily work with individuals in hospice care, a sensitive environment where clients may face advanced illnesses and end-of-life challenges. Your expertise will help these patients and their families navigate emotional and psychological difficulties, ensuring dignity, support, and understanding during a critical time.

Qualifications\n

• A Master’s degree in Social Work is mandatory.

• An active, state-issued LCSW license is required to practice.

• Demonstrated ability to work independently as well as part of a team-oriented environment.

Preferred Experience\n

Compensation and Benefits\n
