Key Responsibilities\n

As a Licensed Clinical Professional Counselor (LCPC) at Crystal Lake, your key responsibilities will include but are not limited to:

• Providing brief, focused interventions for patients requiring immediate mental health support, ensuring alignment with agency protocols and FQHC guidelines.

• Monitoring patient progress through regular follow-ups and adapting treatment plans based on feedback and outcomes.

• Documenting patient interactions and treatment plans in our electronic health record system efficiently to maintain compliance and support continuity of care.

• Collaborating with primary care providers to identify patients at risk and develop comprehensive care plans that integrate mental and physical health services.

• Fostering patient engagement in their own recovery, offering educational resources and self-management strategies tailored to their individual needs.

• Attending team meetings to contribute insights on behavioral health services, outcome evaluations, and program improvements.

• Upholding ethical standards according to the Code of Ethical Conduct for Clinical Professionals and actively participating in professional development.

Qualifications\n

The ideal candidate will possess:
