Key Responsibilities\n

• Conduct comprehensive assessments through interviews and evaluations to determine clients' needs, strengths, and areas of concern.

• Develop and implement individualized treatment plans, setting measurable goals that cater to each client's unique circumstances, including behavioral health challenges related to terminal conditions.

• Facilitate counseling sessions, offering support and guidance that empowers clients to confront and cope with their current challenges.

• Continuously monitor clients’ progress, adapting treatment plans as required to ensure they meet their emotional and psychosocial goals.

• Maintain accurate and up-to-date documentation of client interactions, progress notes, and treatment outcomes in compliance with ethical and legal standards.

• Participate actively in case conferences and contribute insights regarding client welfare, advocating for their needs to ensure comprehensive care.

• Build collaborative relationships with multidisciplinary teams, including medical professionals, hospice staff, and community resources to provide a cohesive support system for clients.

• Assist in connecting clients to additional community services that will aid them in their recovery or provide necessary supports throughout their journey.

This role is centered around providing specialized support to individuals facing end-of-life challenges, as well as their families. Your work will involve addressing a range of emotional responses, including anxiety, grief, and transition processes, in an environment that fosters compassion and comfort. Qualifications\n

• A Bachelor’s Degree in Social Work (BSW) or closely related field is required; a Master’s Degree in Social Work (MSW) is preferred.

• Demonstrated experience in a social work capacity, preferably within a healthcare or hospice environment.

Compensation and Benefits\n
