Key Responsibilities\n

• Conduct thorough assessments of patients to identify emotional, psychological, and social needs, leading to collaborative treatment planning.

• Develop, review, and implement pre-crisis and crisis plans, focusing on behavioral guidelines and positive behavior interventions tailored to individual needs.

• Engage with patients and families to facilitate access to essential community resources, ensuring comprehensive support and advocacy.

• Maintain accurate and timely documentation to comply with regulatory standards, utilizing our electronic health record system to track progress and outcomes.

• Consult with multidisciplinary teams to enhance patient treatment plans, ensuring a holistic approach that addresses all aspects of patient care.

• Actively participate in all regulatory accreditation processes and initiatives that promote the quality of care services.

• Contribute to the development of community partnerships and liaise with state and local agencies to expand resources for our patients.

Patient Population / Client Focus\n

This position primarily focuses on patients in hospice care, who may be grappling with terminal illnesses and the associated emotional challenges. Qualifications\n

• A Master’s Degree in Social Work (MSW) from an accredited institution.

Compensation and Benefits\n
