Key Responsibilities\n

• Conduct thorough psychosocial assessments to diagnose and treat a range of mental health disorders, adapting care plans based on ongoing evaluations of patient status.

• Collaboratively create and implement individualized treatment plans while considering the input and needs of the patient, their families, and other significant stakeholders.

• Refer clients to appropriate community services or treatment programs, ensuring that vital resources such as support groups and psychiatric care are accessible.

• Engage with a multi-disciplinary team to develop comprehensive discharge plans, aiding in the coordination of post-treatment care and community resources.

• Maintain compliance with ethical and legal standards set forth by federal, state, and local guidelines, ensuring adherence to the Mental Health Practice Act.

• Perform social work consultations by offering supportive interventions during times of crisis, such as bereavement or acute emotional distress.

• Educate and communicate effectively across various patient care teams and external agencies, facilitating smooth transitions and resolving any barriers to care.

• Advocate for patients' needs and assist in managing any delays or complex discharge situations to ensure continuity of care.

• Offer professional support and debriefing services to staff involved in high-stress situations in conjunction with our Employee Assistance Program.

This role will cater to a diverse clientele, ranging from children to adults, all facing various mental health challenges. Clients may be dealing with issues such as anxiety, depression, trauma, schizophrenia, and other behavioral health disorders. Qualifications\n

• Master's Degree in Social Work, Counseling, or a related field from an accredited institution.

• Current State Licensure as a Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), or Licensed Professional Clinical Counselor (LPCC).
