Key Responsibilities\n

• Act as a liaison between families and outpatient providers, ensuring that communication is clear and supportive.

• Engage with insurance companies to facilitate pre-certification and manage utilization effectively.

• Contribute to discharge planning and aftercare strategies that address individual client needs and promote ongoing well-being.

• Provide therapeutic meal support, guided by best practices, while demonstrating a healthy relationship with food to clients.

Patient Population / Client Focus\n

This role focuses primarily on older adults facing end-of-life transitions. You will work closely with patients and their families, providing valuable guidance and emotional support that encourages dignity and comfort throughout the hospice journey. Familiarity with the complexities of aging and end-of-life care is essential in this role.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• A valid and active LCSW license in the state of New Jersey.

• Demonstrated experience working with older adult populations, particularly in hospice or palliative care settings.

Compensation and Benefits\n
