Key Responsibilities\n

• Conduct comprehensive assessments to determine the specific needs of clients and develop personalized treatment plans tailored to address their unique circumstances.

• Maintain open communication with relevant parties, including clinical supervisors, family members, and support individuals, to provide updates on patient progress and to collaborate on treatment strategies.

• Organize and manage a flexible schedule of appointments and home visits to monitor client progress, ensuring adherence to the established program plans and objectives.

• Facilitate in-home observations, chart reviews, and staff interviews as part of a holistic approach to client care, integrating various data sources to assess treatment efficacy.

• Participate actively in Interdisciplinary Team Meetings, contributing valuable insights for coordinated care and effective patient management.

• Document client progress through detailed monthly and quarterly progress notes, ensuring compliance with regulatory standards and organizational protocols.

• Provide training and support for team members on the implementation of treatment plans and proper documentation procedures, enhancing team efficacy.

• Act as a liaison to the broader public and mental health community, fostering relationships aimed at promoting mental health initiatives and community well-being.

Qualifications\n

• A Master’s or Doctorate degree in Counseling, Psychology, or a related field is required.
