Key Responsibilities\n

• Conduct comprehensive assessments and create tailored treatment plans that reflect the unique needs and goals of each patient, involving both pre-crisis and crisis planning as needed.

• Document sessions and treatment plans efficiently and accurately, adhering to regulatory standards to ensure compliance and quality of care.

• Utilize evidence-based screening tools as directed by the clinical team to facilitate effective treatment and measure patient progress.

• Collaborate with interdisciplinary teams to devise and implement positive behavior support plans that can enhance patient outcomes.

• Serve as a liaison to various service systems, state agencies, and community partners to enhance service delivery and adaptability.
