Key Responsibilities\n

As a Licensed Professional Counselor, your responsibilities will include:

• Conducting thorough patient intakes, which involve diagnostic and functional evaluations to establish treatment goals and recommendations for the primary care provider.

• Providing brief, focused therapeutic interventions for clients requiring mental health services, ensuring that sessions are aligned with individual needs.

• Collaborating with primary care providers to identify patients who would benefit from continued care in primary settings versus those needing referrals to specialized mental health facilities.

• Initiating follow-up with clients to assess their progress and the effectiveness of current treatment plans, adjusting strategies as necessary.

• Developing relapse prevention plans to assist clients in maintaining stable functioning and preventing deterioration of their mental health conditions.

• Monitoring the delivery of health services related to behavioral health, facilitating connections with external treatment providers with the client’s consent.

• Documenting patient progress and maintaining detailed treatment records in compliance with agency guidelines.
\n
• Needing to express clear communication with the primary care team regarding any changes in patient needs and treatment plans.

• Teaching self-management skills and providing educational resources to empower clients in their recovery journey.

• Acting as a consultant to clinic management regarding behavioral services, helping to evaluate and improve program outcomes.

Qualifications\n

To be considered for this position, candidates must possess:

• A minimum of a Master’s Degree in Clinical Counseling.

• A current and valid LPC (Licensed Professional Counselor) license.

Preferred Experience\n

While not mandatory, candidates who are bilingual in English and Spanish will be given preference, as this skill can significantly enhance client interactions. Additional knowledge of community resources and their applications in mental healthcare will strengthen your candidacy, as well as experience with integrating services for co-occurring disorders.

Compensation and Benefits\n

We offer a competitive salary ranging from $87,000 to $105,000 annually, commensurate with experience and qualifications.
