Key Responsibilities\n

• Conduct thorough assessments to identify patients' psychosocial needs and formulate personalized treatment strategies, including case management, short-term counseling, and crisis intervention.

• Guide patients in completing necessary documentation for entitlements, insurance, and supplemental benefits, helping them transition away from reliance on emergency assistance.

• Provide resources and assistance for accessing medication cost support through various indigent programs and pharmaceutical company initiatives.

• Advocate for patients and facilitate effective communication with community agencies to ensure they receive the support they require.

• Maintain accurate and up-to-date patient records adhering to legal and ethical standards while preparing reports, documentation, and treatment summaries as needed.

• Coordinate logistical support for patient transportation when necessary, ensuring clients can access care without barriers.

• Participate in clinical and team meetings to discuss case management, collaborate on treatment plans, and share insights regarding patient progress and challenges.

Patient Population / Client Focus\n

As a Licensed Mental Health Counselor at Resilience Psychiatry Solutions, you will primarily work with individuals facing various mental health challenges, including anxiety disorders, depression, trauma-related issues, and other psychiatric conditions. Qualifications\n

• A Master’s degree in Social Work, Psychology, Marriage and Family Therapy, or Counseling is required.

• An active license in California as a Licensed Professional Clinical Counselor (LPCC), Licensed Clinical Social Worker (LCSW), or Licensed Marriage and Family Therapist (LMFT).

• A minimum of two (2) years of relevant professional experience providing mental health counseling to clients.
