Key Responsibilities

• Conduct thorough evaluations of patients recovering from injuries, diseases, or surgeries within their homes to develop effective treatment plans.

• Design and execute individualized treatment strategies to meet the specific goals of each patient, ensuring that they attain their highest potential following their rehabilitation journey.
• Diligently document all treatment plans, progress notes, and patient responses to ensure continuity of care and effective communication within the healthcare team.
• Regularly review and reassess therapeutic goals, adjusting plans and approaches as needed to reflect patient progress and challenges.
• Recommend appropriate assistive devices when necessary, fitting patients for equipment that aids in self-care and ambulation.
• Educate patients, families, and caregivers on proper therapeutic techniques and the use of assistive modalities to empower them during the recovery process.
• Maintain current knowledge of advancements in physical therapy practices by attending workshops, participating in professional organizations, and engaging in continuous education.

Patient Population / Client Focus

Your patients will range from children to adults with varying physical therapy needs, including post-operative recovery, chronic pain management, and rehabilitation following sports injuries or accidents. Qualifications

• Bachelor's Degree in Physical Therapy from an accredited institution is required.
