Key Responsibilities\n

• Perform thorough psychosocial assessments on individuals, families, and groups, ensuring a clear understanding of their mental health needs and treatment objectives.

• Conduct initial comprehensive psychosocial evaluations to establish individualized care plans, triaging care based on client urgency and complexity.

• Collaborate with fellow team members and healthcare providers during patient rounds to create integrated treatment strategies aimed at improving client outcomes.

• Maintain accurate, timely, and ethical documentation as required by the center's standards, ensuring compliance with all regulations and facilitating optimal care continuity.

• Implement evidence-based therapeutic modalities, including Cognitive Behavioral Therapy (CBT), to assist clients in progressing toward their treatment goals.

• Lead case management for assigned patients, working closely with community resources and healthcare providers to ensure comprehensive, multidisciplinary care.

Qualifications\n
