OnTrack Physical Therapy & Wellness is actively seeking dedicated and experienced orthopedic physical therapists to join our dynamic team in Fort Myers, Florida. Work Environment

This position will be performed onsite within patients' homes, allowing for a personalized approach to physical therapy. The scheduled hours are Monday through Friday, from 8:00 am to 5:00 pm, giving you a structured work-life balance while ensuring that you can provide consistent care throughout the week. You will collaborate closely with your colleagues and other healthcare professionals to foster a cohesive treatment environment.

Key Responsibilities

• Understand and comply with state practice acts and regulations that govern physical therapy, ensuring the highest standards of ethical and professional practice.
• Conduct thorough evaluations of patients with orthopedic conditions, develop individualized treatment plans targeting their specific functional limitations, and monitor their progress.
• Utilize evidence-based assessment tools to gather pertinent patient history, perform detailed physical examinations, and implement treatment modalities designed for each patient's needs.
• Implement and adapt treatment protocols based on clinical findings, evolving patient needs, and emerging healthcare practices to ensure the best outcomes.
• Collaborate with interdisciplinary teams, recognizing and documenting changes in patient conditions to adjust care plans as necessary.
• Execute comprehensive discharge planning, ensuring all aspects of patient care are documented accurately and timely, including evaluations and interim progress notes.
• Engage with corporate training requirements, maintaining compliance with all regulatory standards while continually enhancing your professional capabilities.
• Take an active role in facility meetings, contributing insights into improving access to therapy services and participating in quality assurance initiatives.

Patient Population / Client Focus

As a dedicated orthopedic physical therapist, you will work primarily with patients experiencing musculoskeletal injuries, post-surgical recovery, and various chronic pain conditions. Your hands-on expertise will make a significant impact on their physical rehabilitation journey, enabling them to regain strength, mobility, and independence over time.

Qualifications

• Doctorate of Physical Therapy (DPT) from an accredited program.
• Active State License as a Physical Therapist in Florida.
• Current CPR certification.
• Strong interpersonal and communication skills to develop rapport with patients and collaborate with healthcare teams.

Preferred Experience

• A minimum of two years of outpatient orthopedic experience, demonstrating proficiency in evaluative and therapeutic techniques.
• Experience in home health settings is preferred but not required.

Compensation and Benefits
