Work Environment

This role is primarily onsite, focusing on home care settings across Fort Lauderdale. Key Responsibilities

• Conduct thorough patient evaluations and assessments to identify functional limitations and develop effective, personalized treatment plans.
• Implement rehabilitation strategies, monitor patient progress, and adjust treatment plans as necessary to optimize outcomes.
• Educate patients and their families on rehabilitation techniques, exercise programs, and recovery strategies to facilitate understanding and compliance.
• Engage in open communication with a multidisciplinary team, including physicians and other healthcare providers, to coordinate patient care effectively.
• Document all treatment sessions accurately and maintain compliance with healthcare regulations and professional standards.
• Participate in community outreach and educational workshops to promote physical therapy services and empower community health initiatives.

Patient Population / Client Focus

Our home care patients often include individuals recovering from surgery, managing chronic conditions, or rehabilitating from injuries. This role allows you to work with a diverse patient population, including the elderly, athletes, and those with developmental disorders. Building rapport and trust is crucial as you provide therapy in their most comfortable environment.

Qualifications

• Bachelor's Degree in Physical Therapy from an accredited program.
• Current state license to practice physical therapy in Florida.
• Basic Life Support (BLS) certification is required.
• Strong written and verbal communication skills, coupled with proficiency in using electronic medical record (EMR) systems for documentation.

Preferred Experience

While a minimum of two years of experience in a clinical setting is required, candidates with home health experience or additional certifications in specialized therapies are highly regarded. Compensation and Benefits
