Work Environment

This full-time position is based onsite, focusing on providing rehabilitative services directly in clients' homes. Key Responsibilities

As a Physical Therapist, you will:

• Direct comprehensive physical therapy treatment tailored to prevent disability, relieve pain, improve muscle function, and maintain maximum performance aligned with the patient's capabilities.
• Oversee, mentor, and collaborate with Physical Therapy Assistants to ensure high-quality implementation of patient care plans.
• Conduct thorough evaluations of clients' functional mobility using diagnostic tests and assessments, adjusting treatment protocols based on findings.
• Provide hands-on therapy which may include manual therapeutic exercises, massage, and the use of physical agents for treatment.
• Collaborate in the development and continuous improvement of care plans based on multidisciplinary team input and client progression.
• Maintain clear and detailed clinical records, including initial evaluations, progress notes, and discharge summaries, ensuring compliance with healthcare regulations.
• Engage in case conferences with interdisciplinary teams, providing insight into treatment effectiveness and patient status.
• Advise clients and families about their therapy goals, treatment devices, and proper usage to facilitate independent living.
• Participate actively in your personal professional development to stay current with industry practices and enhance your impact on patient care.

Patient Population / Client Focus

Qualifications

To be considered for this position, candidates must possess the following:

• A Bachelor's degree in Physical Therapy from an accredited program.
• A minimum of 2 years of experience as a Physical Therapist, preferably in a home care or rehabilitation setting.
• Current licensure and good standing with the Florida Physical Therapy Board.
• Current CPR certification.
• A valid driver's license and reliable transportation to provide home care services.
