Key Responsibilities

• Conduct thorough evaluations to assess patient needs, development of individualized treatment plans based on clinical findings, and documentation of evaluations and progress notes.

• Consult with other healthcare professionals to coordinate multidisciplinary care, identifying any changes in a patient's condition that may necessitate modifications to their treatment plan.

• Implement innovative treatment protocols tailored to the patient's presentation and utilize advanced tools and methodologies to maximize recovery outcomes.

• Educate patients and their families on conditioning exercises, pain management strategies, and self-care techniques to enhance their recovery process.

• Ensure meticulous documentation of patient care activities, including initial evaluations, ongoing progress summaries, and discharge planning, to reflect the patient's treatment journey accurately.

• Participate in continuous training and compliance with regulatory standards to uphold the highest level of clinical practice and patient safety.

• Engage in facility meetings and remain an active participant in team discussions, fostering a collaborative and team-oriented work culture.

Patient Population / Client Focus

Qualifications
