Key Responsibilities\n

• Support patients in managing their mental health needs through comprehensive assessments and personalized counseling plans.

• Utilize evidence-based practices to identify patients at risk for readmission and intervene effectively to prevent it.

• Participate in transition conferences with patients and their families to develop holistic care plans.

• Maintain continuous communication with an interdisciplinary team, ensuring a coordinated approach to patient care.

• Document patient progress meticulously using our electronic medical records system, tracking outcomes and adjusting social work plans as necessary.

• Provide education and resources to families, helping them understand the support systems available for caregivers.

• Advocate for patient rights, ensuring ethical considerations are prioritized while also reporting any vulnerabilities or suspected abuse as mandated by law.

• Facilitate referrals to community resources and health care services, enhancing the overall care continuum for patients.

• Conduct regular follow-ups with an assigned caseload to monitor progress and adapt care plans.

• Maintain updated knowledge of private payer networks and government services to assure smooth transitions for patients.

Qualifications\n

• Possess an independent license in the state of Oregon (LCSW, LMFT, LPC) and a Master’s or Doctorate in clinical psychology, counseling, or social work from an accredited program.
