Key Responsibilities\n

• Conduct thorough, ongoing assessments tailored to individual patient needs and preferences, ensuring effective care plans.

• Maintain open lines of communication with supervisors, caregivers, and other relevant parties regarding patient progress and treatment adjustments.

• Create and share a structured schedule for appointments, home visits (if applicable), and observations to monitor patient advancement.

• Collaborate in interdisciplinary team meetings to share insights, discuss challenges, and brainstorm solutions for various cases.

• Document comprehensive progress notes monthly and quarterly, ensuring accurate records are maintained for each client.

• Organize training sessions for staff to ensure competence in executing the program plan and adhering to documentation practices.

• Act as a resource and liaison for community organizations and stakeholders, promoting mental health support and initiatives.

• Contribute to the development and refinement of training materials to enhance service delivery and patient care.

Patient Population / Client Focus\n

As an LCPC at Wellbeing Counseling Center, you will work with a broad spectrum of clients, from adults dealing with challenges like anxiety and depression to youth struggling with behavioral issues and developmental concerns. Qualifications\n

Compensation and Benefits\n
